Job summary

The Care Team Hospice is seeking a driven Community Care Liaison to grow our presence in the community. You will build relationships with physicians, hospitals, and referral sources to increase awareness and utilization of hospice services.

In this role, you will educate providers, patients, and families on hospice value, guide admissions with empathy, and work toward census targets. Travel is required to serve the assigned geography.

Qualifications

• Bachelor’s degree or equivalent.
• 1–2 years healthcare outside sales experience.
• 1–2 years Hospice or Home Health sales preferred.
• Must have reliable transportation and travel within geography.

Responsibilities

• Build and expand relationships with physicians, hospitals, and referral sources to drive census growth.
• Educate community about hospice value and guide patients and families to care.
• Meet and exceed census/admission targets through partnerships.
• Regularly engage healthcare providers to foster collaborative partnerships.
• Lead discussions with patients and families explaining services with empathy.
