Job summary

The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison to grow our presence in the community. You’ll be the face of our hospice services, building partnerships with physicians, hospitals, and other referral sources to increase awareness and utilization of our services.

Your success will be measured by your ability to grow census through strong partnerships and outreach, educating providers, patients, and families on hospice value, and guiding referrals toward

Qualifications

• Bachelor’s degree or equivalent.
• 1-2 years healthcare specific outside sales experience.
• 1-2 years of Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Educate the community and share the value of comfort-focused, patient-centered hospice care.
• Achieve census and admission targets by positioning hospice as a preferred provider.
