Job summary

Enhabit Home Health & Hospice is seeking a growth-focused professional to manage relationships with physicians, hospitals, and senior living communities. You will educate referral sources about services to drive post-acute care transitions and improve patient outcomes.

The role requires a healthcare business background, strong communication skills, and a valid driver’s license; in-office attendance is essential for team supervision and collaboration.

Qualifications

• Bachelor's degree in healthcare administration or related field.
• Graduate of approved school of nursing, therapy or social work preferred.
• Minimum two years of experience in healthcare business development, community relations, marketing, or sales.
• Understanding of federal, state, and local laws governing Medicare certified home health and hospice operations.
• Valid state driver license and automobile liability insurance as required by law.
• Strong interpersonal, communication and presentation skills.
• Clinical license required if applicable per state rules.
• Experience with home health, hospice, post-acute care, or related services preferred.

Responsibilities

• Serves as the primary relationship manager between the company and referral sources (physicians, hospitals, health systems, senior living communities, etc.).
• Supports referral growth by educating referral sources and patients about services and program requirements to improve patient outcomes and satisfaction.
