Job summary

The Care Team Hospice is seeking a driven, relationship-focused Community Care Liaison to grow our presence in the community. This high-impact role builds referral partnerships with physicians, hospitals, and other referral sources to increase awareness, trust, and utilization of hospice services.

You will be the face of our hospice services in the community, educating providers, patients, and families on the value of hospice and guiding them toward compassionate, high-quality care.

Qualifications

• Bachelor's degree or equivalent.
• 1-2 years healthcare outside sales experience.
• 1-2 years Hospice or Home Health sales preferred.

Responsibilities

• Build and expand relationships with physicians, healthcare partners, and key decision-makers to drive referral volume and market growth.
• Serve as a dedicated advocate for hospice by educating the community and sharing the value of comfort-focused, patient-centered care.
• Achieve and exceed census and admission targets by positioning our hospice as a preferred provider within the community.
• Engage regularly with healthcare providers and referral sources to create strong, collaborative partnerships focused on patient well-being.
• Lead hospice discussions with patients and families, clearly communicating services and guiding the consent process with professionalism and empathy.

Education

Bachelor's degree or equivalent

1-2 years healthcare specific outside sales experience

1-2 years Hospice or Home Health sales Preferred
