Job summary

Enhabit Home Health & Hospice is seeking a qualified registered nurse to lead hospice care coordination. You will oversee patient information, collaborate with physicians, referrals, patients and caregivers, and ensure timely scheduling for field teams to maintain continuous, high-quality care.

Must have an RN license, CPR certification, and at least two years of nursing experience, including hospice or home health. In-office attendance is essential to support supervision and teamwork.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must maintain a valid clinical license as required by state rules.
• Minimum two years of nursing experience.
• At least one year of hospice or home health experience.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification required.
• Valid state driver license and automobile liability insurance.

Responsibilities

• Coordinate hospice services to ensure care is delivered per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care or evaluation visits.
• Facilitate communications between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and maintain care coordination and continuity.
