Enhabit Home Health & Hospice is seeking a nurse leader to oversee the coordination of hospice services, ensuring care delivery aligns with professional standards and company procedures. You will review patient data at key care milestones and act as the liaison between physicians, referrals, patients, caregivers, and field staff.

The role requires a registered nursing license, CPR certification, and at least two years of nursing experience, including hospice or home health exposure.

Qualifications

• Graduate of an approved school of professional nursing.
• Must hold and maintain a valid clinical nursing license per state rules.
• Minimum of two years nursing experience; at least one year in hospice or home health.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification and valid driver's license required.
• Strong technology skills, including use of mobile devices and Microsoft Office.

Responsabilités

• Coordinate hospice services delivered to patients according to professional standards and company procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluation visits.
• Facilitate relationships among physicians, referrals, patients, caregivers, and staff to ensure care continuity.
• Schedule patients to field clinicians and maintain effective communication with all stakeholders.
