Job summary

Qualifications

• Required nursing degree and active RN license.
• Minimum two years of nursing experience in last 36 months.
• Experience with home health preferred.
• CPR certification required.
• Valid driver license and automobile insurance required.
• Strong knowledge of federal, state and local home health regulations.

Responsibilities

• Coordinate home health services for patients per standards and procedures.
• Review patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients with field clinicians and maintain care coordination and continuity.
