Job summary

Enhabit Home Health & Hospice is seeking an experienced nurse leader to oversee hospice service coordination and ensure care delivered meets quality standards.

You will review patient data for start of care and recertification, and facilitate communication among physicians, patients, caregivers, and staff. In-office attendance is essential to supervise teams and drive service excellence.

Qualifications

• Graduate of an approved school of professional nursing and an active nursing license.
• Minimum two years nursing experience; hospice or home health preferred.
• CPR certification maintained; valid driver license and insurance.
• Knowledge of federal, state, and local home care laws and regulations.
• Proficient in Microsoft Office (Word/Excel) and mobile devices.
• Bilingual in English/Spanish is a plus; strong communication skills.

Responsibilities

• Coordinate hospice services per standards and company procedures.
• Review and approve patient information for start of care, recertification, or care evaluation.
• Facilitate relationships among physicians, referral sources, patients, and caregivers.
• Schedule patients to field clinicians and communicate with staff to maintain care continuity.
