The position requires an RN license, CPR certification, and strong knowledge of home health regulations. In-office attendance is essential to supervise, collaborate, and drive optimal patient outcomes.

Qualifications

• Graduate of an approved school of professional nursing.
• CPR certification required.
• Valid state driver license and automobile liability insurance.
• Knowledge of federal, state and local home care laws and regulations.

Responsibilities

• Coordinate home health services per standards and procedures.
• Review and approve patient information for care starts, recertifications, and evaluations.
• Facilitate communication among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and ensure ongoing care coordination.
