Job summary

Enhabit Home Health & Hospice is seeking a qualified RN to lead clinical coordination and ensure high-quality home health services are delivered per standards. The role requires collaboration with physicians, caregivers, and field staff to maintain seamless care.

Requirements include active nursing licensure, CPR certification, and 1–2 years of home health experience. In-office attendance is essential to support supervision, teamwork, and effective service delivery.

Qualifications

• Graduate of an approved school of professional nursing with valid state license.
• CPR certification required.
• Minimum two years nursing experience within the last 36 months.
• Minimum one year of home health experience.
• Knowledge of federal, state and local laws and regulatory guidelines for home care.
• Must have a valid state driver license and automobile liability insurance.
• Demonstrated technology skills including Microsoft Office.
• Preferred bilingual English/Spanish.

Responsibilities

• Coordinate overall provision of home health services to patients per standards and procedures.
• Review and approve patient information during start of care, recertification, or evaluation visits.
• Facilitate coordination among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and maintain clear communication with all stakeholders to ensure continuity of care.
