Job summary

Enhabit Home Health & Hospice is seeking a nurse manager to oversee hospice service coordination, ensuring care aligns with standards and regulations. You will review patient information for start of care, recertifications, and evaluations while fostering collaboration among physicians, referral sources, patients, caregivers, and staff.

The role requires RN licensure, 2+ years of nursing and 1+ year in home health or hospice, plus familiarity with applicable laws.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must be licensed in the state where practicing (Nurse licensure compact rules apply).
• Must have two years of nursing experience.
• Must have one year of home health or hospice experience.
• Must understand federal, state, and local laws governing home care.
• Must have basic technology skills, including operation of a mobile device.

Responsibilities

• Ensure the overall coordination of hospice services provided to the patient is delivered according to acceptable standards of practice and company procedures.
• Review and approve patient information submitted by the licensed professional during a start of care, recertification, resumption of care, or evaluation visit.
• Facilitate the relationship between physicians, referral sources, patients, caregivers, and employees.
