Enhabit Home Health & Hospice is seeking an experienced registered nurse to oversee the coordination of home health services and ensure care aligns with established standards and procedures.

You will facilitate collaboration among physicians, patients, caregivers, and field staff to maintain seamless care delivery and positive patient outcomes. In-office attendance is essential to supervise teamwork and drive quality care.

Qualifications

• Graduate of an approved school of professional nursing and licensed to practice in the state.
• Minimum nursing license in good standing; CPR certification required.
• Experience in home health or comparable care setting preferred.
• Understanding of federal, state and local home health regulations.
• Strong communication and coordination skills with interdisciplinary teams.

Responsabilités

• Coordinate home health services to patients per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption, or evaluations.
• Facilitate collaboration among physicians, referral sources, patients, caregivers, and staff.
• Schedule patient visits and maintain care continuity with field clinicians and providers.
