Job summary

Enhabit Home Health & Hospice seeks a skilled nursing leader to coordinate comprehensive hospice services and ensure care aligns with standards and regulations. You will work with physicians, patients, caregivers, and staff to keep care on track and maintain high-quality outcomes.

The role requires a valid nursing license, several years of nursing experience (including hospice/home health), CPR, and a valid driver license. Excellent communication and tech skills are essential for success.

Qualifications

• Graduate of an approved school of professional nursing with a valid license.
• Minimum two years of nursing experience; at least one year in hospice or home health.
• CPR certification and a valid state driver license; automobile liability insurance.
• Strong knowledge of federal, state and local laws governing home care operations.
• Proficient with technology and Microsoft Office; able to use mobile devices.

Responsibilities

• Coordinate hospice services per standards; review and approve patient information during start of care and recertification.
• Facilitate communication between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and maintain care coordination with all stakeholders.
• Ensure compliance with company procedures and regulatory guidelines.
