Job summary

Enhabit Home Health & Hospice is seeking a skilled Registered Nurse to lead and coordinate comprehensive home health services. You will oversee patient care coordination, review care plans, and liaise with physicians, caregivers, and staff to ensure high-quality outcomes.

The role emphasizes clinical leadership, adherence to regulatory guidelines, and effective communication within a dynamic home care environment. A valid RN license and CPR certification are required.

Qualifications

• Graduate of an approved school of professional nursing and active RN license.
• CPR certification required; valid state driver license and automobile liability insurance.
• Strong communication, organizational and technology skills (MS Office).
• Experience in home health and supervisory/management preferred.

Responsibilities

• Coordinate home health services delivered to patients per standards and procedures.
• Review and approve patient information during start of care, recertification, resumption of care, or evaluations.
• Facilitate collaboration between physicians, patients, caregivers, and staff to ensure continuity of care.
• Schedule patients with field clinicians and communicate across teams to maintain care coordination.
