Job summary
Enhabit Home Health & Hospice seeks an experienced nursing leader to coordinate hospice services for patients in Pasadena, TX. You will ensure care delivery aligns with standards, review patient information, and facilitate coordination among physicians, referral sources, patients, and staff.
The role requires a nursing degree, an active RN license, CPR, and at least two years of nursing experience including hospice or home health.
Qualifications
- Graduate of an approved school of professional nursing.
- Valid clinical license per state requirements, or compact nurse license.
- Minimum two years of nursing experience.
- One year of hospice or home health experience.
- Knowledge of federal/state/local laws for home care operations.
- CPR certification required.
- Valid state driver license and automobile liability insurance.
Responsibilities
- Coordinate hospice services to patients per standards and procedures.
- Review and approve patient information during start of care, recertification, or evaluation visits.
- Facilitate communication between physicians, referral sources, patients, caregivers, and staff.
- Ensure scheduling of patients to field clinicians and maintain care coordination.
