Job summary
Enhabit Home Health & Hospice is seeking a registered nurse to lead care coordination in Houston, TX. You will ensure services meet standards, review care documentation and facilitate collaboration among physicians, patients, and staff.
The role requires an active RN license, CPR, and at least two years of recent nursing and one year in home health. Bilingual English/Spanish is a plus, and strong tech and transportation reliability are expected.
Qualifications
- Graduate of an approved school of professional nursing.
- Valid RN license per state requirements or compact license.
- Minimum two years of nursing experience within the last 36 months.
- Minimum one year of experience in home health.
- Knowledge of federal, state and local home care regulations.
- CPR certification and active driver license required.
- Ability to use mobile devices and basic tech tools.
- Preferred: management experience and HCHB familiarity.
- Preferred: bilingual English/Spanish communication.
- Must have dependable transportation and be able to drive in varied conditions.
Responsibilities
- Coordinate home health services to align with standards and procedures.
- Review and approve patient information at start of care, recertification or care resumption.
- Facilitate collaboration among physicians, patients, caregivers and staff.
- Schedule patients to field clinicians and ensure effective care coordination.
