Job summary

Enhabit Home Health & Hospice is seeking an experienced nurse leader to coordinate hospice services for patients, ensuring care aligns with standards and company procedures. You will review patient information at start of care, recertification, resumption of care, or evaluation visits.

You will also facilitate collaboration among physicians, referral sources, patients, caregivers and staff, and oversee scheduling to maintain seamless care delivery and continuity.

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical license per state requirements.
• Minimum two years nursing experience.
• Minimum one year in hospice or home health.
• Knowledge of federal, state and local laws governing home care operations.
• CPR certification.
• Valid state driver license and automobile liability insurance.
• Basic technology skills including mobile devices.

Responsibilities

• Coordinate hospice services to patients per standards of practice and company procedures.
• Review and approve patient information during start of care, recertification, resumption of care or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers and staff.
• Schedule patients to field clinicians and communicate with field staff, patients, physicians and caregivers to maintain care continuity.
