You will coordinate with physicians, referral sources, patients, caregivers, and staff; manage scheduling and field communications; and support positive patient outcomes through effective operations while maintaining in-office

Qualifications

• Graduate of an approved school of professional nursing.
• Valid clinical nursing license per state requirements.
• Minimum of two years of nursing experience.
• Minimum of one year of hospice or home health experience.
• CPR certification.
• Valid state driver license and automobile liability insurance.
• Ability to use technology including mobile devices and Microsoft Office.

Responsibilities

• Coordinate hospice services to ensure standards of practice and company procedures are met.
• Review and approve patient information during start of care, recertification, resumption, or evaluation visits.
• Facilitate collaboration among physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and communicate with all stakeholders to maintain care continuity.
