Job summary

Enhabit Home Health & Hospice in Houston, TX, seeks a nurse leader to coordinate comprehensive home health services, ensuring care aligns with standards and company procedures.

You will collaborate with physicians, patients, families, and field staff, schedule visits, review start of care, recertifications, and evaluations, and uphold regulatory requirements. This role requires current RN license, at least two years nursing experience (one year in home health), CPR, and valid driver’s license.

Qualifications

• Active RN license in the state of practice.
• Minimum two years of nursing experience within the previous 36 months.
• Minimum one year of home health experience.
• CPR certification maintained.
• Reliable transportation and valid driver license.

Responsibilities

• Coordinate overall delivery of home health services per standards and procedures.
• Review and approve patient information for start of care, recertification, resumption of care, or evaluation visits.
• Facilitate communication between physicians, patients, caregivers, referral sources, and staff.
• Schedule patient visits and ensure care coordination and continuity.
