Job summary

Enhabit Home Health & Hospice is seeking an experienced RN leader to oversee the coordination of home health services, ensuring care aligns with standards and regulatory guidelines. You will review patient information during starts of care and recertifications, while facilitating collaboration among physicians, caregivers and field staff.

The role requires 2+ years of nursing experience in the last 36 months, 1+ year in home health, CPR certification, and a valid state license or compact

Qualifications

• Required graduate of an approved school of professional nursing.
• Current valid clinical license per state requirements or compact license.
• Minimum two years nursing experience within the last 36 months.
• Minimum one year home health experience.

Responsibilities

• Coordinate overall home health services for patients according to standards and procedures.
• Review and approve patient information submitted during start of care, recertification, resumption of care or evaluation visits.
• Facilitate communication among physicians, referral sources, patients, caregivers and staff to maintain care coordination.
