Job summary

Enhabit Home Health & Hospice is seeking a qualified registered nurse to oversee patient care coordination in hospice and home health services. You will review start of care and recertification information, and liaise with physicians, caregivers, and field staff.

The role requires a nursing license, 2+ years of nursing experience, and 1 year in hospice or home health, with strong communication and organizational skills. In-office attendance is essential.

Qualifications

• Must be a graduate of an approved school of professional nursing.
• Must maintain a valid clinical license per state requirements or compact licensure.
• Minimum two years of nursing experience and one year in hospice or home health.

Responsibilities

• Coordinate hospice services delivered to patients according to standards and procedures.
• Review and approve patient information at start of care, recertification, resumption of care, or evaluation visit.
• Facilitate relationships between physicians, referral sources, patients, caregivers, and staff.
• Schedule patients to field clinicians and communicate to ensure care coordination and continuity.
