Job summary

Affinity Health Management is seeking a registered nurse to lead hospice care in patients' homes and communities, leveraging community resources to plan and deliver patient- and family-centered services.

The RN will perform comprehensive assessments, develop and adjust plans of care, administer medications as prescribed, educate families, and collaborate with an interdisciplinary team, including on-call coverage as required.

Qualifications

• Graduate of an accredited school of nursing and valid RN license.
• License in good standing to practice.
• Experience in hospice, public health, or home care preferred.
• CPR certification required.

Responsibilities

• Complete and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Develop and revise plan of care with patient and family as needs change.
• Provide professional nursing care and administer medications as prescribed.
• Coordinate care with the interdisciplinary team and document care.
• On-call duties as defined by policy.
