Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse in good standing to practice.
• 1 year of professional nursing experience within the last 3 years, preferred in home health or hospice.
• CPR certification required.
• Excellent observation, communication, and problem-solving skills.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop, implement, and revise the plan of care with patient and family.
• Administer medications and treatments as prescribed.
• Counsel patient and family on nursing and related needs.
• Coordinate patient care as Case Manager when assigned.
• Maintain accurate clinical notes and communicate changes to the physician.
• Participate in interdisciplinary team meetings and on-call duties.
• Ensure safety and continuity of care in home settings.
• Assist with activities of daily living and support patient self-sufficiency.
