Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize, and direct hospice care for patients in their homes and communities. The role emphasizes community health education and coordinating care based on patient and family needs.

The RN will perform comprehensive assessments, develop and adjust care plans, administer medications, and educate patients and families in self-care and comfort-focused nursing actions.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• RN with a minimum of 1 year of professional nursing experience in the last 3 years; or a BSN from an NLN-accredited program with a current RN license and at least 2 years’ experience (preferably in public health, home care, or hospice).
• Management experience not required; may supervise hospice aides.

Responsibilities

• Complete initial, comprehensive and ongoing patient assessments.
• Provide professional nursing care per the plan of care.
• Coordinate patient care as case manager when assigned by supervisor.
• Teach patient and family self-care techniques and provide instructions as prescribed.
• Participate in on-call duties and ensure equipment and services are available.
• Supervise ancillary personnel as needed and pursue ongoing education.
