Job summary

Affinity Health Management seeks a registered nurse to plan, organize and direct hospice care in home and community settings. The RN will assess patients and families and develop care plans focused on comfort and quality of life.

Role includes delivering nursing care, administering medications, coordinating with the hospice team, and educating patients and caregivers on self-care. State RN licensure and recent hospice/home health experience preferred.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in the state.
• 1 year nursing experience in last 3 years; or BSN with 2+ years preferred.
• Supervisory experience not required; will supervise aides.

Responsibilities

• Completes initial and ongoing comprehensive patient and family assessments.
• Administers medications and treatments per physician's plan.
• Develops and revises care plans with patient and family.
• Coordinates care with the hospice interdisciplinary team.
• On-call duties and equipment coordination as needed.

Education
