Job summary

Affinity Health Management is seeking a Registered Nurse with hospice experience to plan, organize and direct patient care within home and community settings. The role emphasizes comprehensive patient assessment, care planning and coordination with physicians and the care team.

The RN will educate patients and families, administer treatments, and support daily living activities while ensuring safety and continuity of care.

Qualifications

• Graduate of a National League for Nursing accredited nursing program.
• Current licensure to practice as an RN in the state.
• Minimum of two years’ nursing experience (within the last three years) or a BSN with current RN license; public health/home care/hospice experience preferred.
• Management experience not required; may supervise hospice aides.

Responsibilities

• Plan, organize, and direct hospice care for patients in home or community settings.
• Conduct comprehensive patient and family assessments and develop plans of care.
• Provide professional nursing care and administer prescribed medications and treatments.
• Coordinate with physicians and interdisciplinary team; communicate changes in patient condition.
• Educate patients and families on care, self-care, and care plan implementation.
• Participate in on-call duties and ensure necessary equipment and services are arranged.
• Engage in quality improvement activities and professional development.
