Job summary

Affinity Health Management is seeking a Registered Nurse to lead hospice care planning in home settings. You will assess patients, develop and adjust care plans, and coordinate with the interprofessional team to meet patient and family needs.

You will provide nursing care, educate families, and participate in on-call duties, ensuring high-quality, compassionate care within a community-based hospice program.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing.
• 1–2 years of recent professional nursing experience; public health/home care/hospice preferred.
• No management experience required; may supervise hospice aides.
• Subject to references and criminal background check.
• Strong observation, communication, and math skills.
• Must be able to lift and move patients and equipment.
• CPR certification required.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients and families to determine hospice needs.
• Provide professional nursing care per the nursing process.
• Develop and revise care plans with patient and family involvement.
• Act as Case Manager when assigned by Clinical Supervisor; coordinate patient care.
• Maintain accurate clinical notes and communicate with physicians and team.
• Provide patient and family education and support.
• Participate in on-call duties and coordinate equipment and services.
