Job summary

Affinity Health Management is seeking a Registered Nurse to lead hospice home-care services in the community. You will perform comprehensive assessments, develop and adjust care plans, and administer prescribed meds while educating patients and families.

Work with an interdisciplinary team, coordinate with physicians, and participate in on-call duties to ensure compassionate, quality care for patients in their homes and communities.

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• One year of nursing experience in last three years; or a BSN with RN license and two years of relevant public health/home care/hospice experience preferred.
• Supervisory experience not required; may supervise hospice aides.

Responsibilities

• Completes initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process and develops a care plan with patient/family involvement.
• Initiates and revises the plan of care as patient status changes and coordinates with physicians.
• Administers medications and treatments as prescribed; offers patient/family education and self-care guidance.
• Acts as Case Manager for assigned caseload and participates in interdisciplinary team meetings.
• Maintains accurate clinical notes, communicates with physicians, and ensures a safe care environment.
