Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. The RN will lead nursing care with emphasis on community health education and collaboration with an interdisciplinary team.

The role requires comprehensive patient assessments, care planning, and coordination with physicians to ensure high-quality hospice services. Experience in home health or hospice is preferred and licensure in the state is required.

Qualifications

• Graduate of an accredited nursing program.
• Current licensure in the state.
• RN with at least 1 year of professional nursing experience in the last 3 years; or BSN with RN license and 2 years in public health/home health/hospice preferred.
• Supervisory experience not required, but hospice aides may be supervised.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise plans of care with patient and family involvement.
