Job summary

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure in State.
• Registered nurses shall have a minimum of one year of experience; or a BSN with two years of experience in public health/home care/hospice preferred.
• No management experience required.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care using the nursing process and plans of care.
• Writes and initiates plan of care and regularly reevaluates needs.
• Initiates the plan of care and revises as patient status changes.
• Develops a care plan with goals based on nursing diagnosis and family involvement.
• Administers medications and treatments as prescribed.
• Counsels patient and family in meeting nursing needs and self-care.
• Provides health care instructions and supports activities of daily living.
• Acts as Case Manager and coordinates patient care for assigned caseload.
• Completes accurate clinical notes and communicates changes with physicians.
• Maintains safe environment and collaborates with hospice team.
• Participates in on-call duties and quality improvement activities.
