Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize and direct hospice care in home settings, drawing on nursing expertise and community resources. The position emphasizes patient-centered care within families and homes, with a focus on education and comfort.

The RN will assess, plan and implement care, collaborate with the interdisciplinary team, and provide guidance to patients and caregivers while maintaining accurate records and meeting regulatory requirements.

Qualifications

• Graduate of an accredited nursing program with RN license.
• Current RN license in good standing.
• 1 year of professional nursing experience within last 3 years; 2 years with BSN, preferably in home health/hospice.
• Supervisory experience of hospice aides is a plus.
• Valid driver's license with insured vehicle; CPR certification required.
• Excellent observation, verbal and written communication skills.

Responsibilities

• Assess patient and family needs; determine hospice requirements and plan of care.
• Provide direct nursing care per physician's orders using the nursing process.
• Educate patient and family on care, medications and self-care.
• Maintain accurate clinical notes and communicate with physicians and team.
• Participate in on-call duties and ensure equipment/supplies are available.
• Coordinate care with the hospice interdisciplinary team and ensure safe care environment.
