Qualifications

• Graduate of NLN-accredited nursing program.
• Current state RN licensure.
• 1+ year nursing exp within last 3 years; or BSN with RN licensure.
• Experience in public health, home care, or hospice preferred.

Responsibilities

• Complete initial and ongoing comprehensive patient/family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and revise plan of care as patient needs change; initiate revisions as needed.
• Initiate and revise care plans with patient/family involvement.
• Administer medications and treatments per physician's plan.
• Counsel patient and family; provide health instructions and guidance.
• Assist with activities of daily living and comfort care as needed.
• Act as Case Manager when assigned and coordinate patient care.
• Maintain accurate clinical notes; document pain and symptom changes.
• Participate in on-call duties and quality improvement teams.
