Job summary

Affinity Hospice Management LLC in Ohio seeks a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities, leveraging community resources to meet patient and family needs.

The RN will perform comprehensive assessments, develop and revise care plans, administer medications, educate patients and families, and collaborate with an interdisciplinary team to ensure compassionate, high-quality end-of-life care.

Qualifications

• Graduate of NLN-accredited nursing program.
• Current RN licensure in the state.
• Minimum 1 year of professional nursing experience in the last 3 years, or a BSN with RN license.
• Supervision of hospice aides not required but experience is a plus.

Responsibilities

• Complete initial and ongoing patient/family assessments for hospice needs.
• Develop and revise care plans based on nursing diagnoses and patient status.
• Administer medications and treatments as prescribed.
• Educate patients and families on self-care and care plans.
• Coordinate care with the interdisciplinary team and communicate with physicians.
• Participate in on-call duties and ensure safe, compassionate care in home settings.
