Job summary

Qualifications

• Graduate of National League of Nursing accredited school of nursing.
• Current licensure as a Registered Nurse in the state.
• Experience: 1 year as professional nurse in last 3 years or a BSN with RN license; 2 years in public health/home care/hospice preferred.

Responsibilities

• Plan, organize and direct hospice care for patients in homes and communities.
• Perform comprehensive patient assessments and maintain nursing notes.
• Develop and revise care plans with patient and family involvement.
• Coordinate with the interdisciplinary team to meet patient needs.
• Participate in on-call duties and ensure equipment/services are arranged.
