Job summary

Affinity Hospice Management LLC seeks a registered nurse to plan, organize and direct hospice care in Southern Allegheny County. The RN will work primarily in patients' homes, leveraging community resources to meet patient and family needs with a focus on education and quality care.

The role requires an accredited nursing degree, active RN license, and experience in hospice or public health settings. Strong assessment, communication, and care coordination skills are essential for this position.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing.
• 1 year of professional nursing experience within last 3 years; or BSN with 2 years’ experience in public health/home hospice preferred.
• Supervisory experience of hospice aides not required but preferred.
• Satisfactory references and criminal background checks required.
• Excellent observation, verbal and written communication skills; problem solving ability.

Responsibilities

• Completes initial and ongoing comprehensive assessment of patient and family to determine hospice needs.
• Provides professional nursing care utilizing nursing process.
• Assesses status, initiates and revises plan of care as patient needs change.
• Writes plan of care and coordinates with physician.
• Delivers medications and treatments per physician’s orders.
• Counsels patient and family; provides self-care instructions.
• Maintains safe environment and continuity of care across settings.
• Participates in on-call duties and quality/performance improvement activities.
