Qualifications

• Must hold an active RN license in good standing.
• Minimum 1 year of professional nursing experience within the last 3 years; or BSN with 2+ years in public health, home care, or hospice preferred.
• CPR certification required.
• Excellent observation, verbal and written communication skills; problem solving and math basics.
• Must be able to lift and transfer patients and assist with activities of daily living.

Responsibilities

• Performs initial, comprehensive and ongoing patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Writes, initiates and revises the plan of care in collaboration with the physician and family.
• Coordinates patient care as Case Manager when assigned by Clinical Supervisor.
• Maintains accurate clinical notes and communicates with physicians and care team.
