Job summary

Affinity Hospice Management LLC in Southern Allegheny County is seeking a Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. The ideal candidate brings nursing experience and a commitment to community health education.

You will assess patients and families, develop and revise care plans, administer medications, counsel on self-care, and collaborate with an interdisciplinary team.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• 1 year of professional nursing in last 3 years; or BSN with 2+ years in health/home/hospice preferred.
• Experience supervising hospice aides not required.
• Background checks and references required.
• Excellent observation, verbal and written communication skills; nursing skills per competency checklist.
• Ability to lift and position patients; physical stamina for walking/standing.
• Licensed driver with insured vehicle; CPR certification required.

Responsibilities

• Completes initial and ongoing patient/family assessments to determine hospice needs.
• Provides professional nursing care using the nursing process.
• Develops and revises the plan of care as patient status changes.
• Initiates plan of care and coordinates care with the interdisciplinary team.
• Administers medications and treatments as prescribed and counsels patients/families.
• Maintains accurate clinical notes and reports changes to physicians.
• Participates in on-call duties and ensures necessary equipment and services.
• Fulfills case assignments and participates in quality improvement activities.
