Job summary

Affinity Hospice Management LLC seeks a dedicated Registered Nurse to plan, organize and direct hospice care in patients' homes and communities. You will assess patients, develop care plans, and coordinate with physicians and the interdisciplinary team to meet medical and supportive needs.

The role emphasizes community health education, patient/family counseling, and driving quality outcomes in a home-based hospice setting. Strong nursing skills and CPR certification are required.

Qualifications

Graduate of an accredited school of nursing.
RN license in good standing to practice.
BSN preferred with 2+ years of public health/home care/hospice experience.

Responsibilities

Perform initial and ongoing comprehensive patient/family assessments.
Develop and revise plans of care with patient/family involvement.
Provide professional nursing care using the nursing process and prescribed treatments.
Counsel patients and families, educate on care plans and self-care techniques.
Coordinate care and communicate with physicians and interdisciplinary team.
Participate in on-call duties and ensure required equipment and services are available.
