Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will apply nursing processes to assess, plan and provide patient-focused care with emphasis on family education.

The role includes coordinating care with the interdisciplinary team, participating in on-call duties, and ensuring safe, compassionate hospice services. Qualifications include RN licensure, CPR, and 1–2 years in hospice/public health preferred.

Qualifications

• Graduate of an accredited school of nursing.
• Licensed RN in good standing to practice.
• RN with 1 year of experience in the last 3 years or BSN with 2 years in public health/home hospice preferred.
• Management experience not required; supervises hospice aides.
• Offer contingent upon references, as requested, and criminal background checks.
• Excellent observation, verbal and written communication, problem solving, basic math skills; nursing skills per competency checklist.
• Prolonged walking/standing; able to lift and transfer patients and equipment.
• Must be a licensed driver with insured vehicle.
• Possesses and maintains CPR certification (hands-on).

Responsibilities

• Complete initial, comprehensive and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Assess and evaluate patient status; write plan of care and revise as needed.
• Initiate and revise the plan of care as patient needs change.
• Use health data to determine nursing diagnosis and care goals.
• Develop care plans including family in planning; implement palliative actions.
• Administer medications and treatments as prescribed.
• Counsel patient and family; provide health instructions.
• Assist with activities of daily living and promote self-sufficiency.
• Act as Case Manager when assigned and coordinate patient care.
