Job summary

Affinity Health Management is seeking a registered nurse to plan, organize and direct hospice care in Southern Allegheny County. The RN will build on community resources to serve patients in home and community settings, emphasizing education and quality end-of-life care.

The role requires nursing licensure, strong communication, assessment and care coordination skills, with experience in hospice or public health preferred.

Qualifications

• Graduate of an accredited school of nursing.
• RN licensed in good standing to practice.
• Experience in public health, home care or hospice nursing preferred.

Responsibilities

• Perform comprehensive initial and ongoing assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient, family, and interdisciplinary team.
• Administer medications and treatments as prescribed.
• Counsel patients and families and provide health education.
• Coordinate patient care as Case Manager when assigned.
