Job summary

Affinity Health Management is seeking a registered nurse to lead hospice care in the Southern Allegheny County area. The role focuses on planning, organizing, and directing home-based hospice services with emphasis on community health education and patient-family involvement.

The RN will perform comprehensive assessments, develop and revise care plans, administer medications, counsel families, and collaborate with the clinical team to ensure quality of care and patient comfort.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing.
• Minimum 1 year of professional nursing experience (last 3 years) or BSN with 2 years’ experience in public health, home care, or hospice preferred.
• Management experience not required.
• Background checks may be required; references may be requested.
• Excellent observation, verbal and written communication, problem solving, and basic math skills.

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments for hospice needs.
• Provide professional nursing care using the nursing process and develop a care plan.
• Initiate and revise the plan of care as patient status changes.
• Administer medications and treatments as prescribed; counsel patient and family.
• Coordinate care as Case Manager and work with the interdisciplinary team.
• Complete clinical notes and communicate with physicians and community providers.
• Participate in on-call duties and supervise ancillary personnel as required.
• Facilitate patient self-care and ensure a safe home care environment.
