Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will work closely with patients and families to develop and implement individualized care plans, administer medications, and educate on symptom management.

The role requires strong nursing assessment skills, CPR certification, and the ability to lift and transfer patients. Driving to patient homes is expected as part of this field-based position.

Qualifications

• Graduate of an accredited school of nursing.
• Registered Nurse with license in good standing to practice.
• Minimum 1 year of professional nursing experience within the last 3 years or 2 years with public health/home care/hospice preferred.
• CPR certification required.
• Valid driver's license with insured vehicle.
• Excellent observation, verbal and written communication skills.
• Able to lift, position and transfer patients; standing for long periods.

Responsibilities

• Complete comprehensive patient and family assessments to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise the plan of care with patient/family involvement; administer medications as prescribed.
• Educate patient and family; provide health instructions and ensure safe environment.
• Serve as Case Manager for assigned caseload; attend interdisciplinary meetings.
• Participate in on-call duties; arrange equipment and services; supervise aides.
• Maintain accurate clinical notes; coordinate with physician; ensure continuity of care.
