Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• Experience as a professional nurse within the last three years or a nursing degree with two years in public health/home care/hospice.

Responsibilities

• Complete initial, comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care using the nursing process.
• Develop and revise care plans in collaboration with the inter-disciplinary team.
• Coordinate patient care as case manager when assigned.
• Participate in on-call duties and ensure equipment and services are arranged.
