Job summary

Affinity Hospice Management LLC seeks a Registered Nurse to plan, organize, and direct home-based hospice care. You will assess patients and families, develop and revise care plans, and provide nursing care per physician orders while guiding caregivers in self-care and comfort measures.

You will work with an interdisciplinary team to ensure continuity of care, maintain accurate clinical notes, and advocate for patient comfort and dignity in home environments.

Qualifications

• Graduate of an accredited nursing program.
• Licensed RN in good standing.
• Minimum one year of professional nursing experience in home health/hospice or related area preferred.
• CPR certification.
• May require driving and use of a personal vehicle.

Responsibilities

• Assess patient and family to determine hospice needs and plan of care.
• Provide professional nursing care using the nursing process.
• Maintain and communicate care plans and notes with the interdisciplinary team.
• Counsel patients and families on nursing and related needs.
• Assist patients with daily living activities and promote self-sufficiency and comfort.
