Job summary

Affinity Health Management is seeking a Registered Nurse to plan, organize and direct hospice care in home and community settings. You will assess patients, develop and revise plans of care, and coordinate with physicians and families.

The role emphasizes community health education, safety, and collaboration with an interdisciplinary team. A valid RN license and CPR certification are required, with BSN preferred and strong communication skills.

Qualifications

• Graduate of an accredited nursing school and must be a licensed RN.
• RN license in good standing to practice.
• BSN plus 2 years’ experience preferred, with at least 1 year in public health, home care or hospice.
• Supervising hospice aides is part of the role; no management experience required.
• Background checks and references are required.
• Excellent observation, verbal and written communication, problem solving and math skills.
• Must have a valid driver’s license and reliable automobile.
• CPR certification required (hands-on verification).

Responsibilities

• Perform initial and ongoing comprehensive patient and family assessments to determine hospice needs.
• Provide nursing care using the nursing process and plan of care.
• Develop and revise care plans with patient/family involvement.
• Administer prescribed medications and treatments.
• Educate patients and families on care plans and self-care.
• Act as Case Manager for assigned caseload and coordinate patient care.
• Maintain accurate clinical notes and communicate with physicians.
• Ensure a safe environment and continuity of care with the interdisciplinary team.
• Participate in on-call duties and quality improvement activities.
