Job summary

Affinity Hospice Management LLC is seeking a Registered Nurse to plan, organize, and direct hospice care in patients' homes and communities. The role emphasizes community health education and coordination with families and the interdisciplinary team.

The nurse will perform comprehensive assessments, develop and revise care plans, administer medications, counsel patients and families, and participate in on-call duties to ensure continuous, quality care.

Qualifications

• Graduate of an accredited school of nursing.
• RN license in good standing to practice.
• Experience in public health, home care or hospice preferred.
• Excellent observation, verbal and written communication skills; problem solving.

Responsibilities

• Perform initial, comprehensive assessments of patient and family to determine hospice needs.
• Provide professional nursing care following the nursing process.
• Develop and revise the plan of care with patient and family involvement.
• Administer medications and treatments prescribed by the physician.
• Educate patient and family on care techniques and health management.
• Act as case manager and coordinate patient care for assigned caseload.
• Participate in on-call duties as defined by policy.
• Maintain accurate clinical notes and communicate changes to the team.
