Job summary

VitalCaring Group in Tomball, TX is seeking a Care Transition Coordinator to connect patients, families, and healthcare partners with hospice services. This field-based role blends healthcare sales, care coordination, and partnership development to support patient outcomes and organizational growth.

You will serve as the primary liaison between referral sources and clinical teams, drive referral growth, and ensure timely admissions with strong relationship-building and CRM documentation.

Qualifications

• Bachelor’s degree in Healthcare, Business, Marketing, or related field preferred (or equivalent experience).
• Minimum of two years healthcare or related sales experience.
• Strong communication, relationship-building, and influencing skills.
• Ability to manage multiple priorities in a fast-paced, field-based environment.
• Proficiency with Microsoft Office and CRM systems.
• Demonstrated ability to work independently and drive results.
• Valid driver’s license and reliable transportation.

Responsibilities

• Serve as the primary liaison between referral sources, patients, caregivers, and clinical teams to ensure seamless care transitions.
• Execute structured sales calls including pre-call planning, relationship building, needs discovery, and closing.
• Build and maintain relationships with referral partners through consistent communication and service excellence.
• Develop and implement strategies to grow market share and drive referral volume.
• Track and document sales activity across referral sources using CRM/EMR systems.
• Achieve established referral and admission goals.
• Collaborate with hospitals, physicians, case managers, and community stakeholders to support timely admissions.
