St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive—Hospitals to build strong referral relationships with hospitals and healthcare partners, and educate teams about hospice care.

You will drive admissions, coordinate marketing activities, and work closely with clinical staff to ensure timely, compassionate referrals while supporting families through the transition. Strong communication and strategic planning are required.

Qualifikationen

• Associate degree in marketing, business admin, healthcare, or related field preferred.
• 3+ years in a sales role, preferably hospice, medical device, or pharmaceutical sales.
• Strong communications, negotiation, and public relations skills.
• Ability to pass DHS background study.

Aufgaben

• Meet admission and call volume targets.
• Coordinates daily sales and marketing operations including implementation of marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider of services.
• Collaborate closely with clinical partners to develop and execute strategies that drive patient referrals and business growth.
• Document all sales activity in the CRM per organizational requirements.
• Mentors and supports Care Transition Team; participate in territory management.
