Job summary

St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive to build strong hospital referral relationships and educate on hospice care. The role focuses on hospitals, coordinating intake for timely referrals and supporting marketing initiatives.

You will collaborate with clinical partners, manage marketing events, and contribute to budget planning while driving patient referrals and growth across assigned territories.

Qualifications

• Associate’s degrees in marketing, business administration, healthcare, or related field preferred.
• 3+ years of proven success in a sales role, preferably within hospice, medical device, or pharmaceutical sales.
• Demonstrates good communications, negotiation, and public relations skills.
• Autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Ability to pass DHS background study.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor customer perceptions of St. Croix Hospice as a high-quality provider of services.
• Collaborate with clinical partners to develop and execute strategies that drive patient referrals and business growth.
• Document all sales activity in the CRM per organizational requirements.
• Assist the Director of Patient Services/Regional Director in budgeting and marketing allocations.
• Monitor allocation of resources according to budgetary limitations.
• Mentor and shadow Care Transition Coordinators to assist in territory management.
