Job summary

St. Croix Hospice is seeking a Care Transition Coordinator/Account Executive - Hospitals to develop and maintain referrals from hospitals and communities. You will educate on hospice care, assist intake for timely referrals, and collaborate with clinical partners to drive patient referrals and growth.

You will coordinate marketing initiatives, monitor volumes, and support budgeting while building strong relationships with referral sources and care teams.

Qualifications

• 3+ years of proven success in a sales role, preferably hospice, medical device, or pharmaceutical sales.
• Demonstrates strong communications, negotiation and public relations skills.
• Ability to position services with tact and professionalism.
• Autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities.
• Ability to pass DHS background study.

Responsibilities

• Meet admission and call volume targets.
• Coordinate daily sales and marketing operations including marketing initiatives.
• Build and monitor perceptions of St. Croix Hospice as a high-quality provider.
• Collaborate with clinical partners to drive patient referrals and business growth.
• Document all sales activity in the CRM per organizational requirements.
• Employ marketing and promotional initiatives to achieve budgeted sales volume projections.
• Maintain knowledge of markets, government agencies, payer groups, referral sources, and competitors.
• Enhance communication among patients, family, caregiver, facility, physician team, and hospice vendors.
